Sapphire at Sweet Home is located in Sweet Home, Oregon, offering companionship care and cognitive support. We are a behavioral support and care community designed to provide optimal comfort, care and privacy to our residents.

Our carefully selected staff are available to support each resident with the activities of daily living such as dressing, bathing and management of medication according to their plan of care.
Each resident is encouraged to select their own agenda and activities, while feeling secure in knowing that our staff are there when needed. Each resident and the interdisciplinary care team develop a plan of care that aligns the care with the desired outcomes.

In working closely with community mental health specialists, we provide residents and families with the peace of mind that our goal is to enhance the life of every person we serve.
